(2) Documents for Identity Verification

In order to prevent leakage of personal information to a third party impersonating the customer or unauthorized falsification of personal information, a document to verify the customer's identity (hereinafter referred to as an "identity verification document")is required when making a request.Please send a copy of your driver's license, health insurance card, or passport an identity verification document together with the request form in (1).In all cases, the portion with your name and address is required.

(3) Required documents for requests by proxy

If the person making the request is a person with parental authority or a guardian of a minor, an assistant, a conservator, or an adult guardian under the legal guardianship system, or a voluntary guardian under the voluntary guardianship system (collectively, "legal representative"),or a representative authorized by the person, please send the following documents in addition to the identity verification documents described in (2).

Person with parental authority：A copy of a family register, or a copy of an insurance card with dependents listed
Guardian of a minor：A copy of the person's family register

A copy of an identity verification document for the legal representative (same as (2))

2 In the case of an authorized representative

A copy of the power of attorney (any format)
A certificate of the person's seal impression (issued within the last 3 months)
A copy of an identity verification document for the authorized representative (same as (2))

(4) The applicable fee

The fee for disclosure of personal data in our possession, or notification of the purpose of use is as follows.
Handling fee：¥840 (tax included) per request
Payment method：Please enclose ¥840 worth of postage stamps alongside the application form.

(5) Method of response to requests

We will respond to your request in writing to the address indicated on the request form.

(6) Actions to be taken in case of incomplete documents, etc.

In the following cases, the Company will contact you to inform you that you need to resubmit or add documents, etc.If the items specified by the Company (the resubmission of documents, etc.) are not completed (including cases where they do not reach the Company) within two weeks after having contacted you, the Company will treat the request as if it had not been made.Please note that even in such cases, the prescribed fee will be charged.

(7) Purpose of use for personal information obtained in connection with a request

Personal information obtained by the Company in connection with a request for disclosure, etc., will be handled only to the extent necessary to respond to the request, including investigation and contact regarding the request.
